if you are an agency support worker seeking a referral for your client - complete the below form

Cafs delivers a range of community support programs, some of which only take direct referrals from an agency support worker from partnering agencies. 

This referral form can be used for the programs listed on the Cafs Program Information and Eligibility Page.

If you work in Community Services and would like to refer a client to one of Cafs support programs please complete the Intake Referral Form below and a member of our Cafs team will be in touch with you as soon as possible.

If you would like to consult with Cafs team member regarding a referral for a Cafs program, please contact Cafs Client Engagement Team on 1800 692 237.

 

If you are having technical problems using the online form, please download the Client Engagement Intake Referral Form  here and  return the completed form to: welcome@cafs.org.au

 
* Please note that some Cafs programs do not take direct referrals from the community.
